Online learning FAX IN REGISTRATION

By the Book Learning FAX IN REGISTRATION

To REGISTER by fax: Choose this option if you have not registered with TLC yet this term and you are interested in either online or By the Book training.

Click the the appropriate fax link above.  You will be taken to a form that must be filled out andfaxed in.

  1. By clicking in the first blank field, the form can be filled in using your keyboard.
  2. Alternatively, the form can be printed out and filled in using a pen.
  3. All fields are required. Your registration will be returned, unprocessed if information is omitted.
  4. The form should be printed and faxed to 352-955-6872 or 955-7000or truck mailed to TLC at the Sivia Center.
  5. Your records will be updated within 48 hours of the time your registration is received.
  6. TLC will fax a confirmation back to you for your records.

 

Option B-->ONLINE REGISTRATION: (currently down. Please use Option A)

Choose this option if you have not registered with TLC yet this term. When you click the online registration link, you will be taken to a new registration form. Have the following information handy before entering the registration page:

  1. Center Number: Your Center Number is a 4-digit school or center number.
  2. E-mail address: Please use your ACPS e-mail address if possible.
  3. Home/emergency telephone: In the event that a class must be cancelled at the last minute.
  4. Class ID and Class Name: Each class has a relative Class ID and Name. Refer to the TLC calendar for this information.
  5. Username and Password: A username and password is required to submit the registration online. Your username and password are:
  1. Submitting a new registration: By clicking the "Submit" button your registration is automatically entered into the TLC database.
  2. Confirmation: Print that final screen for your records. You will not receive any other confirmation from TLC.
